The Blood Disorders Center at the University of Colorado Hospital is transforming health care for our malignant blood cancer and benign hematology patients by not only providing exemplary patient-centered care but also through world-class clinical research trials and our revolutionary partnership with the CU School of Medicine in the development of the CARE Innovation Center, a comprehensive service that brings together health care and leading industry and start-up partners. The Blood Disorders Center services patients with Lymphoma, Leukemia, MDS, and Multiple Myeloma, as well as rare blood disorders such as Castleman’s Disease, Hairy Cell Leukemia and Langerhans. Some of these patients will proceed to either an Autologous or Allogeneic Bone Marrow or Stem Cell Transplant or CAR-T Cell Therapy. We also see patients with Myeloproliferative Neoplasms such as Polycythemia Vera, Myelofibrosis and Essential Thrombocytosis as well as other non-malignant blood disorders, including anemia, Sickle Cell Disease, and Thalassemia.

The Clinical Social Worker plays an integral role within the Blood Disorders program. This person will be responsible for working closely with our fantastic team of Psychologists, Physicians, Advanced Practice Providers, Nurses, Pharmacists, Dieticians and Transplant/CAR-T Coordinators to help navigate the complex care coordination for our patients. A blood cancer diagnosis carries with it many issues and needs that the Social Worker can assist with, from counseling patients on how to cope with their diagnosis and treatment to helping provide financial and community resources that the patient and caregiver may not otherwise be aware of. The Social Worker assesses patients and caregivers and plans interventions to help them cope with social, emotional, economic and environmental problems. Working closely with the Psychology team, the Social Worker will be key to identifying and helping break down healthcare barriers.

The Social Worker will participate in case reviews to evaluate care and will consult with other healthcare members to promote, monitor and evaluate compliance with treatment plans. This person will play a key role in discharge planning and processes, helping to identify appropriate resources such as lodging, transportation, social and/or spiritual services and financial assistance programs. The Social Worker will administer and interpret psychosocial functioning tests to help diagnose mental, emotional, behavioral, addictive and developmental disorders and disabilities and then work alongside our team to implement appropriate treatment plans.

The Clinical Social Worker is such an important member of our team and is truly key to helping our Blood Cancer patients thrive and succeed despite their devastating diagnosis. 

Summary:

Assesses patients and/or patient's family, caregivers, and/or legal representatives and arranges for needed interventions. Plans interventions to help patients cope with social, emotional, economic, and environmental problems and provides short term treatment planning for mental, emotional and behavioral disorders, conditions and addictions.

Responsibilities:

  • Participates in case reviews to evaluate care management and progress toward goals. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Periodically administers and interprets tests and measures of psychosocial functioning for the purpose of diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities, implementing appropriate assessment-based treatment plan.
  • Supervises and/or trains new staff, lower level social workers, students or interns.

Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

    Essential:
    * Licensed Clinical Social Worker

    Minimum Required Education: Master's degree in Social Work.
    Required Licensure/Certification: State licensure as a Licensed Clinical Social Worker (LCSW).
    BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixty days of hire. All life support certification cards must be issued by either the American Heart Association or the American Red Cross CPR for the Professional Rescuer. (American Red Cross for the Professional Rescuer is only good upon hire and all renewals will need to be through an American Heart Association sanctioned course.)

The pay range for this position is: $31.61 - $47.41/ hour. Pay is dependent on applicant's relevant experience.
