Summary:

Licensed Clinical Social Worker (LCSW) - Emergency department 
Organizes and coordinates the activities and flow of work for the transition of complex Care patients. Reduces the overall Length of Stay, decrease readmission, and ensure a safe disposition for patients.


Work Schedule: Full Time Days

 

Responsibilities:

  • Lead Social Worker Complex Care Clinician in the Emergency Department
  • Provides mentoring and guidance to Clinicians around skilled communication, education, and resources that promotes quality and cost-effective outcomes.
  • Organizes and coordinates the activities and flow of work for Social Services. Serves as the subject matter expert for the Social Work care continuum process.
  • Act as a resource to the Social Work team and is the first point of escalation for frontline team members regarding complex cases and patient needs
  • Participates in leadership meetings and planning sessions. Designs, evaluates, and recommends program and policy development.

Requirements:

  • Licensed Clinical Social Worker (LCSW)
  • Minimum Required Education: Master's degree in Social Work.
  • Required Licensure/Certification: State licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum Experience: 1 year of experience at the Licensed Clinical Social Worker level

The pay range for this position is: $33.82 - $50.73/ hour. Pay is dependent on applicant's relevant experience.
