Care Coordination Specialist, MACC (Medicaid Accountable Care Collaborative) and Healthy Harbors Programs & Team, UCHealth Community Health Improvement department, UCHealth-North / Fort Collins-based. 32 hours per week

The Care Coordination Specialist with the MACC & Healthy Harbors programs and team provides intensive, community-based care management services and support to program clients who have complex medical and/or behavioral health care needs. MACC & Healthy Harbors care coordinators also help to connect program clients and families to relevant community resources while addressing social determinants of health and barriers to accessing care.

Potential client focus areas for this specific position may include working with high-risk obstetrics (high-risk pregnancy) patients, patients struggling with addiction and / or mental illness, and patients who experience adverse health impacts due to instability with social determinants of health, esp. related to lack of housing / homelessness. Extensive knowledge of local community resources and experience working in a care management capacity with the above-noted client populations is preferred. 


  • Minimum Required Education: Bachelor's degree in Public Health, Nutrition, Exercise Science, Health, Wellness, Social Work, or related field.
  • Minimum Experience: None Required. Preferred: Experience working with under-served populations or in community health programs.

The pay range for this position is: $22.80 - $29.64 / hour. Pay is dependent on applicant's relevant experience.
