CeDAR, the Center for Dependency, Addiction, and Rehabilitation, is a division of UCHealth serving patients in both inpatient and outpatient settings. The main CeDAR campus located on the UCHealth Anschutz academic medical center campus in Aurora, CO and we operate a small outpatient clinic in Boulder as well. Summary:Performs services in-person or virtually involving milieu management, assessment, planning, and implementation of therapeutic and educational services for patients in the inpatient, emergency department or ambulatory setting.
Work Schedule: PRN Flex
Facilitates assigned patient assessments, triages for appropriate clinical consultation, individual and group therapeutic activities including coaching milieu management, and case management and care coordination
Systematically and continually assesses the patient's coping mechanisms in collaboration with the patient, care team and family.
Utilizes appropriate teaching methods and/or referrals to ensure patient/family psychoeducational needs are met.
Assesses, collaborates and coordinates patient's discharge planning needs and readiness for discharge with other members of the care team.
Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
The pay range for this position is: $20.87 - $27.13 / hour. Pay is dependent on applicant's relevant experience.
