Manager Post-Acute Care Management
Responsible for liaising between UCHealth and post-acute care (PAC) settings. Oversees and evaluates quality-based metrics for attributed lives within UCHealth and value-based contracts to identify areas for performance improvement. This may be a “hybrid” position…approximately 70% work-from-home…but you must reside in Colorado.

Job duties

· Responsible for establishing and monitoring processes to identify, track and trend metrics of the program and identify intervention, evaluate documentation and outcomes. Determines appropriateness of interventions and evaluates for quality of care. Coordinates activities related to patient flow and assists with strategy goals.

· Manages staff relations including performance management, staff satisfaction and conflict management. Performs and oversees scheduling, recruitment, payroll and student engagements.

· Monitors departmental budgets, regulatory compliance, departmental contracts and vendor relations. Determines and justifies needs for systems/equipment/supplies purchases, monitors usage, and oversees proper working order and/or stock supplies.

· Provides oversight and coordination for ambulatory and care setting participation in Medicare Share Savings Plan (MSSP) and Clinically Coordinated Care (C3).

Requirements

  • Bachelor's degree in Nursing.
  • State licensure as a Registered Nurse (RN).
  • 2 years of related experience.
  • 10 years of Post-Acute leadership experience.

 

The pay range for this position is: $47.57 - $76.11 / hour.
