Pharmacy Medication Safety Coordinator

Summary:

The Medication Safety Coordinator is responsible for coordinating all activities related to medication safety in collaboration with pharmacy staff, medical staff, nursing staff, and other pertinent health care providers. These activities involve reporting, system and process evaluation, monitoring of activities related to medication safety, formulation of recommendations and initiatives, and coordination of education. The Medication Safety Coordinator works to assess medication use system processes, identify opportunities to improve medication use safety and review, design solutions for system failures, and institute practice changes. Knowledge of the concepts and application of safety principles as well as continuous quality improvement will be imperative to success in this role. The Medication Safety Coordinator should have a passion for improving patient care and enhancing the organization’s culture of safety.

Work Schedule: Full Time, 80 hours/pay period, Days

Responsibilities:

  • Coordinate a consistent and timely approach to medication-related event review and follow-up. Utilize data driven performance improvement methodologies and tools to develop recommendations for action.
  • Collaborate with multidisciplinary teams to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures.
  • Maintain a working knowledge of trends and developments in medication safety through continuous professional development. Utilize information from the Institute for Safe Medication Practices (ISMP), the Food and Drug Administration (FDA), the National Medication Error Reporting Program, the MedWatch program, etc.
  • Facilitate the development of system changes that promote the safe use of medications. Implement and evaluate programs and/or initiatives, including medication use evaluation studies and medication safety programs.
  • Lead multidisciplinary teams concerned with medication errors, adverse drug events and reactions, near misses, and patient safety to identify risk points and prioritize system improvements to reduce the potential for medication errors and patient harm.
  • Serve as a resource to staff regarding departmental/area policies, procedures, services, and regulatory compliance. Precept pharmacy students and residents as assigned. May have supervisory responsibilities.
  • Within scope of job, requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Requirements:

  • Minimum Required Education: Bachelor's Degree of Pharmacy from an accredited school of pharmacy.
  • Required Licensure/Certification: State licensure as a Pharmacist.
  • Minimum Experience: 2 years post graduate experience.
  • Preferred: Completion of an ASHP accredited PGY1 residency program and a PGY2 in medication-use safety or at least three years of equivalent hospital based experience.

The pay range for this position is: $54.31 - $81.47 / hour. Pay is dependent on applicant's relevant experience.
