The Clinical Social Worker plays an integral role supporting both the Breast Center as well as the Multispecialty. This person will be responsible for working closely with our fantastic team of Psychologists, Physicians, Advanced Practice Providers, Nurses, Pharmacists, and Dieticians to help navigate the complex care coordination for our patients. The Social Worker assesses patients and caregivers and plans interventions to help them cope with social, emotional, economic and environmental problems. Working closely with the Psychology team, the Social Worker will be key to identifying and helping break down healthcare barriers.
The Social Worker will participate in case reviews to evaluate care and will consult with other healthcare members to promote, monitor and evaluate compliance with treatment plans. This person will play a key role in helping to identify appropriate resources such as lodging, transportation, social and/or spiritual services and financial assistance programs.
