Thrombosis and Hemostasis Clinical Pharmacist


Summary

Coordinates and provides comprehensive medication management to improve prescribing, dosing, and monitoring of anticoagulation, anticoagulation reversal, and clotting factor concentrates.


Responsibilities

Under the supervision of the Critical Care Clinical Pharmacy Supervisor, the Thrombosis and Hemostasis Pharmacist will be responsible for developing extensive anticoagulation reversal and complex anticoagulation services, including a factor stewardship program for the University of Colorado (UCH) Hospital. This program will be developed in collaboration with UCH providers, including but not limited to benign hematologists. As part of this program, the formulary agents available at UCH will be reviewed and updated. Guidelines outlining blood factor prescribing, order review, compounding, and administration will be established. The Thrombosis and Hemostasis Pharmacist will serve as the factor product expert at UCH and will be responsible for initial and ongoing education of pharmacy, provider, and nursing staff. The scope of this position will apply to all inpatient areas, emergency department, operating rooms, and outpatient areas. The Thrombosis and Hemostasis pharmacist will be critical in establishing transitions of care plans to ensure safe and effective prescribing practices. In addition, it will be expected for the Thrombosis and Hemostasis Pharmacist to round with the benign hematologists daily and provide treatment plans and dose modifications as appropriate.


Minimum Qualifications

Doctor of Pharmacy from degree from an accredited school of Pharmacy required. State licensure in Colorado as a Pharmacist required. Completion of a PGY2 residency in a related specialty area preferred OR completion of PGY1 residency with at least five years of experience in benign hematology.
