The MCC Cancer Care & Hematology Clinic is in the North Medical Office Building at the Medical Center of the Rockies in Loveland. Clinic hours are currently M-F 0800-1700. The clinic is currently closed on weekends and holidays and is not open overnight. The RN Care Manager position is an advanced nursing role that focuses on symptom management and prevention of hospitalizations of patients undergoing cancer treatment. You will work closely with the interdisciplinary team to help coordinate quality care for the patients. This position is non-exempt and will be 24 hours per week/ 48 hours per pay period. Schedule will be consistent 8 hour shifts each week with some variability depending on the staffing need. Oncology experience and certification is preferred

Summary:
Works collaboratively with physicians, interdisciplinary teams, individual clients and families to promote positive client outcomes. Performs a care continuum process that assesses, plans, implements, coordinates, monitors and evaluates the options and services required to support the individual’s health needs utilizing skilled communication, education, and resources to promote quality, cost-effective outcomes. Provides focused support to various areas such as utilization management, emergency department, acute, ambulatory and specialty care.

Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

The pay range for this position is: $36.47 - $56.52 / hour. Pay is dependent on applicant's relevant experience.
