Shift: Days 

Pay: $69.65 - $111.44 / hour. Pay is dependent on applicant's relevant experience

Summary:
Directs the strategic, growth, and operational objectives for the Care Management service line in conjunction with organizational strategic plans. Directs and oversees continuous improvement initiatives, utilizing an inter-disciplinary and multi-departmental approach, to achieve quality and superior patient care and services across the continuum of care. Handles system data governance.

Responsibilities:

• Develops and directs strategic, growth, and operational objectives for the service line. Integrates inpatient and outpatient services and strategic plan with the mission, vision, and values of UCHealth.
• Supports and leads from a process improvement perspective, utilizing benchmarking data to continually achieve higher levels of excellence and cost effectiveness. Maintains staffing as volumes change and works to identify opportunities to modify systems and processes to further improve quality or reduce costs. 
• Directs and oversees program development, performance management, personnel activities, budget, and the provision of services. Systematizes reporting and implements operation data-driven accountability.
• Oversees department operations. Develops and empowers management teams to lead daily operations, resolve issues, manage employee performance, manage physician relations, ensure high quality patient care and service excellence, and ensure operational efficiencies.
• Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Requirements:

• Minimum Required Education: Master's degree in Nursing, Healthcare or relevant field.
  Required Licensure/Certification: None required. Preferred: Case Management Certification.
  Minimum Experience: 5 years of relevant experience.
