Pay: $31.61 - $47.41 / hour. Pay is dependent on applicant's relevant experience
Summary:
Coordinates the care of patients with chronic respiratory disease.
Responsibilities:
Coordinates patient assessment and care, including collaborative care, case management, education, and discharge.
Participates in multi-disciplinary meetings for the purpose of managing/reviewing patient care and treatment protocols, policies, and procedures.
Serves as a resource to staff in areas of program development, clinical practice, and professional development.
Assists with the development of educational materials for patients. Organizes and facilitates support groups for patients and their families.
Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.
